When I visit this website, do you collect information on where I go and what I do?
During your visit to DVCPS.COM information that is not readily identifiable to an
individual is automatically collected and stored. This information will be used
to collect statistical data in the number of visitors, the pages they visit and
for how long. For a more detailed discussion of our privacy policy, please select
the Privacy Notice link on the bottom of each page in
the PUBLIC AREA.
What’s on this website?
This website is divided into two major areas: the PUBLIC area and the STUDY PARTICIPANTS area. The Study Participants area is password-protected and requires that you review and digitally sign a Participant Informed Consent form and register, i.e. username, password, and email address.
Why do I need to register?
Visitors to this site do not need to register. However, potential study participants do need to register to participate in this study.
Why do you need my email address?
The truth is I have a lot of friends out there who really want my study to be successful.
So these good friends of mine may try to take the two surveys over and over again.
We use your email address, along with other data to prevent my friends from . .
. well . . . stuffing the survey boxes. The other reason, I need your email address
is to notify you in case you win one of the $100 Ebay certificates for your organization.
Anyone who completes both surveys is eligible to win!!!
Okay, I get why you need my email address, but why do you need my name – my
real name?
Well there are these people at the University of Phoenix who belong to this group
called the Institutional Review Board, IRB for short. They’re real sticklers about
those pesky little human subjects’ protections, like making sure you understand
what you’re getting into in terms of this study (that’s the informed part). So you
have to sign a Participant Informed Consent Form using your REAL name, acknowledging
that you are participating in the study of your own free will (that’s the voluntary
part).
Uh, did I miss something? How are you going to get my real name signature?
Okay this part is really cool. You sign your Participant Informed Consent Form digitally,
online with just the click of the mouse!!
Will people be able to see my personal information or survey responses?
No!!! Absolutely not!! Well except me. I need to see your survey responses to make
sure each survey is complete. I need to check each survey so I can prepare them
for data analysis. However, I won’t know if it’s YOUR survey because there are no
personal identifiers on the survey instruments – just a number. Because of the nature
of the research design, your participant code (number) is used to link your training
and attitude surveys, registration form, prescreening survey and Participant Informed
Consent Form together. For example, a participant code tells me that participant
#0001 has a signed an informed consent form, completed both surveys, and registered
in the Ebay raffle. I don’t know is the name of participant #0001, just that the
individual attached to that number completed the study process.
How does the process work?
You must register to access the survey instruments. Registration means reviewing and digitally signing a Participant Informed Consent Form, providing your real name, selecting a username and password and providing an email address. Your Informed Consent Form is linked to your real name. I have to review all consent forms. However, I will not be able to see your name. All I will see is a code number that was assigned to you when you registered to participate in the study. Your code number is linked to your consent form and survey instruments.
In the event, that I need to review a specific Informed Consent form, I will pull up a special code sheet that is located in a pass-word protected file on my hard drive (offline). When the code sheet file is open, I have to perform additional actions in order to view the code-name match list. The integrity of my computer system, files, and Internet Service is protected by Norton Internet Security. Each day, automatic security updates are downloaded. My hard drive is continuously being scanned for security breeches. There is additional security software to ensure that my system is fully protected.
My computer savvy is limited to sending and receiving email. What if I need
help navigating the site?

What if I’m in the middle of taking one of the surveys and I get called away
by a crisis?
No problem. The website is your friend. Hit the logoff button. The computer will
ask you if you’re sure. It does that because the Internet has been conditioned to
believe that humans are fickle. When you return to the website, login with your
username and password and you’ll be right back to where you ended when you logged
off. To protect your privacy, your registration information is only valid for 7
days. After that it gets erased. Sorry, but I am committed to protecting your privacy.
What if I decide at some point that I no longer wish to participate?
No problem. You can quit at anytime. If you want to quit before the 7 days expires,
send me an email at dvparsignup@dvcps.com
and I will have your personal information deleted. You will receive a follow-up
email indicating that it has been deleted. To verify that your information has been
deleted, simply go to the website and enter your username and password. You should
receive a message that says, “no one found matching that username and password.”
What if I still have questions or just don’t feel comfortable taking an online
survey?
If you still have questions send me an email at
dvparsignup@dvcps.com with a phone number so we can talk. If you’re uncomfortable
with online surveys, you can email me about that too and maybe together we can figure
out what the discomfort is about. However, in the end, it has to be your choice
to participate. Which ever way you decide, I will support your decision. If you
decide that it’s not a good fit for you – no problem. If you decide you want to
try it – fine. The most important thing to remember is that it has to be YOUR CHOICE,
not mine.

What are you going to do with the results from this study?
Well, I’d like to present my findings at a national forum. Then I’d like to publish
the results so other scholars and researchers can build on what I’ve learned.
